Mindset Social Work Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our full-time team in providing critical support to patients in our hospice care setting. This remote position allows you to deliver impactful mental health services while offering flexibility in your schedule, including some evening and weekend hours to cater to patient needs. Work EnvironmentAs a remote social worker in hospice care, you will operate within a supportive, interdisciplinary team that includes nurses, physicians, and other mental health professionals. Key Responsibilities- Conduct thorough psychosocial assessments of patients and families, gathering vital information about their psychological, social, and environmental factors that impact well-being.
- Provide individual, family, and group therapy sessions, utilizing techniques such as cognitive-behavioral therapy, solution-focused therapy, or trauma-informed care to promote healing.
- Design and implement tailored treatment plans that outline specific goals and interventions adapted to the unique circumstances of each client.
- Engage in crisis intervention when necessary, performing comprehensive risk assessments and establishing safety plans for patients in distress.
- Maintain meticulous and timely documentation in alignment with ethical and legal standards to ensure continuity of care and compliance.
- Facilitate psychoeducational workshops and support groups, bringing together patients and families to strengthen community ties and provide additional resources.
Your role will center around a diverse group of patients in hospice care, including individuals diagnosed with terminal illnesses and their families. You will address a wide variety of mental health concerns, such as grief, anxiety, and depression, all while fostering an environment of compassionate support. The goal is to empower patients and their loved ones to navigate the emotional challenges of serious illness with resilience and dignity.
Qualifications- A Master’s degree in Social Work from an accredited institution.
- Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
- Strong foundation in evidence-based therapeutic techniques and interventions appropriate for hospice care settings.
- A minimum of two years of experience in a mental health or outpatient setting, preferably in hospice or palliative care.
- Experience working with patients dealing with grief and loss is highly desirable.
- Demonstrated ability to communicate effectively with patients, families, and interdisciplinary team members.
- Proficiency in electronic health record (EHR) systems for documentation and care coordination.
