Clinical Social Worker - Hospice Care

Bliss Reformation and Wellness Center is seeking a dedicated Clinical Social Worker to join our compassionate team in a remote role focused on hospice care. The ideal candidate will hold a current state licensure (LCSW) and possess a deep commitment to enhancing the emotional and psychosocial well-being of our clients, particularly seniors facing end-of-life transitions. This full-time position offers an annual salary ranging from $90,000 to $100,000.

This position is designed to be primarily remote, with occasional on-site visits required for assessments and meetings. You will work collaboratively with a multidisciplinary team that includes healthcare professionals, caregivers, and administrative support to ensure seamless service delivery. The typical work hours for this position are Monday through Friday from 8:00 AM to 4:30 PM, with the understanding that flexibility is essential. There may be occasional evening or weekend hours based on client needs, and on-call availability will be necessary to address urgent situations.

• Conduct comprehensive psychosocial assessments to evaluate the emotional, mental, and social needs of senior clients.


• Develop and implement personalized care plans in collaboration with clients and their families, focusing on their specific goals and preferences.


• Provide robust counseling and emotional support, helping seniors navigate significant life transitions, health-related challenges, and emotional distress.


• Act as an advocate for client rights and access to community resources, ensuring effective navigation of available support systems.


• Regularly visit clients in their homes to monitor overall well-being and assess living conditions while addressing any safety or care concerns.


• Facilitate family meetings and discussions to enhance communication among family members and resolve potential conflicts in care decisions.


• Educate clients and their families about available local resources, support groups, and programs designed to improve their quality of life.


• Maintain meticulous documentation of client assessments, care strategies, progress notes, and interactions to promote continuity of care.


• Stay abreast of evolving social work practices and relevant policies to ensure compliance and ethical practice in client interactions.

This position primarily focuses on elderly clients in hospice care, who often cope with complex emotional and psychosocial challenges related to aging, chronic illnesses, and end-of-life issues. We aim to create positive therapeutic relationships that empower clients and their families, helping them to find peace and dignity in their circumstances. • A Master’s degree in Social Work (MSW) from an accredited institution is required.


• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).


• Strong understanding of social work principles, particularly those related to geriatric care and hospice services.


• Excellent assessment, counseling, and communication skills, with a proven ability to interact empathetically with seniors and their caregivers.


• Ability to work effectively in a multidisciplinary team setting, collaborating with healthcare professionals to ensure comprehensive care.


• Proficiency in utilizing electronic health record (EHR) systems and other digital tools for documentation and client management.

• A minimum of two years of clinical social work experience, preferably within a community health setting or geriatric care environment.


• Demonstrated experience in providing support for individuals facing life-limiting illnesses and their families.


• Familiarity with local community resources specific to the Miami area.
