Key Responsibilities

  • Conduct virtual assessments to understand each client's unique emotional, psychological, and social needs, developing tailored intervention strategies.

  • Formulate, implement, and regularly update individualized treatment plans, aligning services with best practices in hospice care.

  • Actively advocate for clients' rights and their access to essential resources, including healthcare services, community support, and financial aid.

  • Engage in collaborative meetings with interdisciplinary teams, including physicians, nurses, and other therapists, to ensure comprehensive, holistic care strategies are in place.

  • Provide emotional support and counseling to clients and their families, helping them navigate the challenges of end-of-life decisions and experiences.

  • Utilize electronic health record systems to document interactions and progress efficiently, ensuring compliance with legal and ethical standards.

  • Create and facilitate workshops or support groups aimed at enhancing coping strategies and emotional resilience among clients and families.

Patient Population / Client Focus

In this role, you will primarily support patients and families involved in hospice care, a setting characterized by its focus on palliative treatment and end-of-life care. You will work closely with individuals experiencing complex emotions related to grief, loss, and adjustment, ensuring they receive compassionate support tailored to their cultural and psychological needs. Your expertise will be critical in advocating for holistic approaches that enhance the quality of life during challenging times.

Qualifications

  • A Bachelor's Degree in Social Work from an accredited program is required.

  • Strong written and verbal communication skills, with an emphasis on empathy and understanding in diverse situations.

  • Knowledge of hospice-specific regulations, practices, and patient rights.

Compensation and Benefits
