Key Responsibilities

As a Licensed Clinical Social Worker, your responsibilities will include:



  • Serving as a vital liaison between families and outpatient providers, facilitating clear communication and coordinated care.

  • Working directly with insurance companies to manage pre-certifications and utilization reviews, ensuring our clients receive timely services.

  • Involvement in discharge and aftercare planning to guarantee seamless transitions for clients as they move through different stages of care.

  • Providing therapeutic meal support that models and encourages healthy relationships with food.

  • Offering safe, empathetic, and affirming care that addresses each client’s unique psychological needs and intersecting identities.

Patient Population / Client Focus

Our client base includes individuals with varying needs, with particular emphasis on older adults experiencing complex emotional and psychological challenges related to health, grief, and transitions in end-of-life care. As an LCSW at Vision Works, you will develop meaningful therapeutic relationships, addressing the nuanced demands of your clients while championing their unique narratives and dignity.

Qualifications

The ideal candidate will meet the following qualifications:



  • A Master’s degree in Social Work from an accredited institution.

  • A valid LCSW license in the state of Illinois.

  • Demonstrated experience working with older adults, particularly in hospice or healthcare settings.

  • Strong clinical and diagnostic skills that enable effective assessment and intervention planning.

  • Excellent communication and interpersonal skills for effective collaboration within a multidisciplinary team.

  • Ability to work independently while maintaining a strong connection to team objectives.

Preferred Experience

While not mandatory, candidates with the following experience will be prioritized:



  • Prior involvement or training in grief counseling or palliative care.

Compensation and Benefits
