Registered Nurse - Skilled Nursing & Rehabilitation

• Conduct thorough assessments of residents' health status.
• Administer medications and manage treatment plans effectively.
• Support rehabilitation objectives and collaborate on personalized care strategies with a multidisciplinary team.
• Monitor and document any changes in residents' conditions promptly in the electronic medical record (EMR) system.
• Provide education and support to residents and their families regarding care plans and health management.

• Current Registered Nurse license.
• Strong assessment and clinical skills.
• Ability to work collaboratively within a team.
• Excellent communication and interpersonal skills.
