Job Description
A medical facility located near Louisville, KY is seeking a full-time Director of Case Management to join its team. Director of Case Management ready to join a community of individuals passionate about what they do need apply! Details are below.

Pay: $100,000 - $120,000/Year

Director of Case Management Opportunity:

  • Full-time permanent position

  • Director of Case Management manage the daily operations and HR functions of the Case Management department

  • Directs the interdisciplinary care, discharge planning, and patient outcomes while championing patient advocacy

  • Guides and trains staff, builds relationships with healthcare and insurance networks, and fosters a positive, celebratory work culture

Director of Case Management Qualifications:

  • Must be a licensed clinician (ex: RN, SW, OT, PT, ST, Rehabilitation counseling, etc.)

  • Current CCM or ACM certification or eligibility to obtain within first year of employment

  • Must possess a bachelor's degree; graduate degree is preferred

  • Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience
