As a remote Social Worker, you will regularly interact with patients, families, and the clinical team via virtual platforms, utilizing various telehealth tools and methodologies to facilitate your care delivery.

Key Responsibilities
  • Psychosocial Assessment: Conduct timely assessments utilizing your theoretical knowledge of human behavior and psychosocial factors affecting treatment outcomes, while recognizing potential ethical and risk-related issues.
  • Clinical Interventions: Provide compassionate supportive counseling to patients and families, assist in navigating community resources, facilitate family meetings, and address difficult decisions regarding care options such as hospice and Do Not Resuscitate (DNR) orders.
  • Case Load Management: Manage a varied clinical caseload effectively, identifying high-risk situations and collaborating with Case Managers on discharge planning to ensure seamless transitions of care.
  • Legal and Ethical Standards: Uphold legal and ethical guidelines, addressing issues such as suspected abuse, conservatorship, and patient rights, ensuring adherence to all Tenet Policies.
  • Documentation: Maintain accurate and comprehensive documentation in line with organizational policies, including progress notes and treatment plans, ensuring the highest standards of patient care.
  • Educational Initiatives: Deliver in-service training to hospital staff on psychosocial aspects of patient care, contributing to our community’s knowledge base through educational sessions like Lunch and Learns.
  • Quality Improvement: Engage in process improvement initiatives, evaluating and analyzing patient care indicators to enhance service delivery.
  • Team Collaboration: Work collaboratively with a multidisciplinary team, mentoring graduate students and providing supervision to Master of Social Work (MSW) employees as needed.
Patient Population / Client Focus

The primary focus of this role is to serve a diverse patient population, including those facing complex health issues and psychosocial challenges. You will encounter patients from various backgrounds, including those dealing with chronic illnesses, mental health issues, and unique end-of-life care situations. Your ability to communicate effectively and empathetically will directly influence their treatment experiences and outcomes.

Qualifications
  • A Master's degree in Social Work from an accredited graduate school.
  • A valid California LCSW license.
  • A minimum of 2 years of experience in a medical setting, with at least 1 year in a clinical cancer care environment.
  • Outstanding analytical, problem-solving, and planning skills to assess and meet patient needs.
  • Exceptional verbal and written communication abilities to engage with healthcare teams and patients.
  • Proficiency in managing complex cases independently while thriving as part of a cohesive team.
Preferred Experience
  • Experience in community health interventions or multidisciplinary team settings.
  • Familiarity with electronic health record (EHR) systems and telehealth platforms.
  • Background in leading support groups or educational programs tailored for patients and families.
Compensation and Benefits

This position offers a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience and qualifications.
