Seeking a dedicated and highly skilled Nurse Practitioner for a specialized primary care and case management department focusing on the 65+ population. This is an opportunity for a provider who is passionate about improving outcomes for seniors through coordinated, comprehensive care.
Key Responsibilities
Clinical Oversight: Provide and oversee primary care services tailored to the unique needs of geriatric patients.
Care Coordination: Lead a team of case managers to develop, implement, and monitor individualized care plans.
Regulatory Excellence: Ensure all clinical activities meet Florida state regulations and federal healthcare standards.
Interdisciplinary Collaboration: Work closely with physicians, families, and specialized staff to ensure seamless care transitions and holistic support.
Process Improvement: Identify opportunities to enhance patient satisfaction and clinical efficiency within the department.
Required Qualifications
Education: Masters Degree in Nursing (MSN) from an accredited program.
Licensure: Current, active Nurse Practitioner (APRN) license in the state of Florida.
Experience: At least 1 year of professional nursing experience (RN).
1 3 years of experience practicing as a Primary Care Nurse Practitioner.
Specific expertise or background in geriatric care (65+) is required.
Skills: Strong clinical diagnostic skills, leadership presence, and proficiency with Electronic Medical Records (EMR). read more
