Hospice Medical Social Worker - Brooklyn, NY

Compass Healthcare Consulting & Placement is conducting a search for a Hospice Medical Social Worker position making visits to patients at home in Brooklyn, NY. Qualified candidates will have prior Hospice or Home Care Social Worker or bereavement counseling experience as well as the ability to travel to patient locations. Opening available within Brooklyn, NY. Driver's license and access to vehicle, required.

The Hospice Social Worker provides medical social services, psychosocial and bereavement counseling to the hospice Patients and families at home. Position requires travel. Visit areas available in Brooklyn, NY.

Qualifications:

• Minimum one (1) year bereavement counseling experience is preferred.


• At least one year of Hospice or Home Care experience preferred.


• New York State driver’s license required.


• Excellent interpersonal, and communication skills.


• Exhibits an ability and attitude relative to the spirit of caring as required by specific job duties and functions.

Education:

• MSW from an accredited school of social work and one year related experience.


• LMSW or LCSW required.
