Project Manager, Clinical Quality Intervention




Location: Chicago, IL


Work Environment: Hybrid (3 Days In Office)


Employment Type: Full-Time


Schedule: Day Shift


Hour: $37/hr-$40/hr


Funding: Grant-Funded Position Through December 2026




Position Overview




We are seeking an experienced Project Manager, Clinical Quality Intervention to lead clinical quality improvement initiatives focused on enhancing hypertension control and improving patient outcomes across partner primary care clinics.


This role will serve as a key liaison between healthcare providers, clinic leadership, community organizations, and program stakeholders to ensure the successful implementation, monitoring, and sustainability of quality improvement programs. The ideal candidate brings a strong background in healthcare project management, public health initiatives, quality improvement, and stakeholder engagement.




Key Responsibilities




Project Management & Clinical Quality Improvement



  • Lead the planning, coordination, and execution of clinical quality improvement initiatives across multiple partner clinics.

  • Develop and manage project timelines, milestones, budgets, and deliverables.

  • Monitor project performance and implement solutions to address challenges and maintain progress toward goals.

  • Support the implementation of evidence-based interventions designed to improve hypertension management and clinical outcomes.

  • Ensure project activities align with grant objectives, timelines, and reporting requirements.




Stakeholder Engagement & Collaboration



  • Serve as the primary point of contact for clinic partners, healthcare providers, community organizations, and program stakeholders.

  • Facilitate communication, collaboration, and engagement among participating organizations.

  • Build strong relationships with clinical teams to support adoption of quality improvement practices.

  • Coordinate meetings, working groups, and stakeholder discussions.




Data Management & Reporting



  • Oversee data collection, quality assurance, analysis, and outcome tracking activities.

  • Monitor key performance indicators and quality metrics.

  • Prepare reports, presentations, and progress updates for leadership, partners, and funders.

  • Ensure accuracy, completeness, and integrity of project data.




Training & Program Sustainability



  • Support training and capacity-building activities for clinic staff and healthcare teams.

  • Promote best practices in quality improvement and population health management.

  • Assist with sustainability planning and integration of successful interventions into routine clinical operations.

  • Identify opportunities for process improvement and long-term program impact.




Compliance & Grant Management



  • Ensure compliance with grant requirements, regulatory standards, and organizational policies.

  • Maintain documentation and records required for audits, evaluations, and reporting.

  • Support continuous quality improvement efforts and program evaluation activities.




Required Qualifications



  • Bachelor's degree in Public Health, Healthcare Administration, Nursing, Health Sciences, or a related field.

  • 3-5 years of project management experience in healthcare, public health, population health, or community-based programs.

  • Experience managing project timelines, deliverables, budgets, and stakeholder relationships.

  • Experience supporting quality improvement, care management, population health, or clinical initiatives.

  • Strong analytical, organizational, and problem-solving abilities.

  • Excellent written and verbal communication skills.
