Personal Care Aide / Home Health Aide

SagePoint Healthcare LLC is seeking a reliable and compassionate caregiver/home health aide for a client in the Springfield, OH 45502 area.

This is a part-time position offering 24 hours per week with a consistent schedule:

Thursday, Friday, and Saturday: 12:00 PM – 8:00 PM

Responsibilities

• Assist with activities of daily living and personal care as authorized in the client’s service plan
• Provide homemaking and light housekeeping assistance
• Assist with meal preparation
• Provide companionship and support
• Help maintain a safe and comfortable home environment
• Follow the client’s care/service plan and SagePoint policies
• Accurately document services provided and comply with EVV requirements
• Report changes in the client’s condition or concerns to the appropriate supervisor

Qualifications

• Dependable, compassionate, and professional
• Ability to follow a service plan and agency procedures
• Reliable transportation to the client’s home
• Must successfully complete all required pre-employment screening, background checks, training, and onboarding requirements before assignment
• Previous caregiving, home care, STNA, HHA, or similar experience is preferred
