Work Environment

The role is situated within our state-of-the-art assisted living facility, where our focus is on delivering personalized care to elderly residents and individuals requiring assistance with daily living activities. The facility is designed to provide a comfortable and supportive atmosphere, allowing residents to maintain a sense of independence while having access to comprehensive medical support. As part of our team, you will work Monday to Friday from 9 AM to 5 PM, in an environment that encourages innovation and teamwork.

Key Responsibilities

• Conduct thorough health assessments and physical examinations for residents, addressing both acute and chronic health issues.
• Diagnose medical conditions and formulate treatment plans in collaboration with physicians and other healthcare team members, ensuring the best possible patient outcomes.
• Order and interpret diagnostic tests, such as blood work or imaging, to support clinical decision-making.
• Prescribe medications and therapeutic interventions in accordance with established protocols, monitoring the effectiveness and adjusting treatment plans as necessary.
• Educate residents and their families regarding treatment options, preventive care measures, and health maintenance strategies.
• Document all patient interactions and care plans diligently within our Electronic Medical Record (EMR) system to maintain accurate and retrievable healthcare records.
• Participate in care team meetings to discuss patient care strategies and continuous improvement measures.

Patient Population / Client Focus

This role specifically focuses on the elderly population residing in our assisted living facility, many of whom may face complex health challenges and require a specialized approach to care. The Nurse Practitioner will be integral in fostering relationships with residents, understanding their unique health care needs, and developing individualized care plans that enhance their quality of life.

Qualifications

• A Master's Degree or Doctorate in Nursing from an accredited institution.
• Current Registered Nurse (RN) and Nurse Practitioner (NP) license in New York State or eligibility to obtain.
• National board certification from a recognized body (such as AANP or ANCC) is required.
• Current BLS and ACLS certifications or willingness to obtain them upon hire.
• Strong clinical judgment skills with the ability to make independent decisions.
• Excellent communication and teamwork skills, essential for working collaboratively within a dynamic multidisciplinary team.

Preferred Experience

• At least two years of experience as a Nurse Practitioner, ideally within an acute care or hospital setting.
• Experience working with geriatric populations or in assisted living facilities is highly valued.
• Previous engagement in interdisciplinary care teams and care planning.

Compensation and Benefits

The Nurse Practitioner position offers a competitive salary ranging from $180,000.00 to $200,000.00 per year, along with a comprehensive benefits package that includes:
