Job Description

Why You’ll Love Working With Us

At AMM/SMG, we think of you first. We care for our patients like family, and we’re committed to culturally attuned, patient-centered care. On our value-based care (VBC) team, you’ll make sure providers have the right information at the right moment so every patient gets thorough, personal care, while building in-demand skills in risk adjustment and quality measures.

What You’ll Do

  • Records & documentation: Retrieve external records from hospitals, labs, imaging centers, and specialists, then upload and tag them in the EHR to support care coordination and risk adjustment.

  • Pre-visit planning: Prepare pre-visit checklists covering care gaps (HEDIS/Stars), due screenings, HCC risk conditions requiring MEAT documentation, and prior hospitalizations or ER visits. Confirm eligibility, health plan attribution, and consents with the front desk and care coordination teams.

  • Annual Wellness Visits (AWVs): Prepare AWV packets (HRAs, PHQ-9, Mini-Cog, and Advance Care Planning forms), pre-load EHR templates, and flag any missing or overdue items.

  • Quality support: Identify patients due for preventive screenings (e.g., colorectal, breast cancer, diabetic eye exam, A1c), prepare reminders, and assist with supplemental data submissions to payors.

  • Clinical support: Perform intake and rooming (vitals, medication reconciliation, chief complaint), administer immunizations or point-of-care tests per standing orders if credentialed, and assist providers as needed.

What You’ll Bring

  • bilingual English/Korean

  • High school diploma or equivalent and completion of a certified Medical Assistant program

  • 1+ year of experience in a primary care or clinical office setting

  • Familiarity with EHR systems (e.g., eClinicalWorks, Athena, Epic) and strong knowledge of medical terminology and documentation workflows

  • Excellent organization, attention to detail, and communication skills, with the ability to work independently and on a team in a fast-paced clinic

  • Preferred: experience with value-based care, Medicare Advantage, HCC documentation, or AWVs; knowledge of HEDIS, Stars, and HCCs

What We Offer

  • Full medical benefits

  • Paid time off (PTO) and paid holidays

  • 401(k) contribution
