Hospice RN Case Manager
Home Care | Monday–Friday, 8:00 a.m.–4:30 p.m.
Serving Marlborough, Framingham, Holliston, and Franklin
Compensation: $93,600–$97,760 per year, based on experience

Support patients and families through the hospice journey with skilled, compassionate care. You’ll assess patient needs, create and coordinate individualized care plans, and help ensure comfort and quality of life.

What we are looking for

• At least one year of experience in an acute care setting or comparable work

• Oncology and home care experience preferred

• Hospice experience preferred
