Description

American Traveler is seeking a Physical Therapist for a Home Health position in Virginia, requiring at least one year of acute care experience and a valid VA PT license.

Details

• Home health setting covering Loudoun and Fairfax counties in Virginia
• Full-time schedule, Monday through Friday, 8:00 AM to 5:00 PM
• One weekend per month on-call rotation required
• HomeCare HomeBase charting system experience preferred but not required

Requirements

• Minimum of one year of experience as a PT in an acute care setting required
• Home care experience preferred
• VA PT license required if applicable
• ACLS certification preferred
• Valid driver's license and auto insurance with candidate listed as a driver required
• Strong communication skills with patients, families, physicians, and co-workers
• Knowledgeable in therapy best practices and sound clinical judgment
• Detail-oriented with disciplined time management and prioritization skills

Additional Information

• Responsibilities include patient assessment, evaluation, documentation, discharge planning, and development of short- and long-term goals
• Utilizes assistive devices and equipment such as crutches, gait belts, walkers, and wheelchairs
• Provides home exercise programs, patient and family education, and safety/injury prevention guidance
• Familiarity with OASIS, plan of care documentation, and Medicare/Medicaid billing practices beneficial
• Mileage reimbursed for professionals located in Virginia
