Primary Coverage Need: Nutrition & Health Coaching

• The primary responsibility will be one-on-one nutrition and health coaching appointments.
  

Coaching Schedule:

• 
  
• Monday Admin
  
• Tuesday: 8:30 AM – 4:00 PM
  
• Wednesday: 10:30 AM – 5:00 PM
  
• Thursday: 8:30 AM – 4:00 PM
  
• Friday Admin
  

Six-session coaching program:

• 
  
• Each session is 30 minutes.
  
• Coach will report Onsite, but all coaching can be conducted virtually.
  
• Members schedule their own appointments. These appointments would likely be conducted via Microsoft Teams or telephone.
  
• Minimal administrative coordination is required.
  

Focus areas

• General wellness and nutrition coaching
  
• Chronic condition management (e.g., diabetes, hypertension, hyperlipidemia, weight management)
  

License/Certification:
