Altru Rehabilitation Hospital seeks a compassionate Case Manager to coordinate comprehensive rehab services for inpatient and outpatient populations. In this role, you will manage a caseload from admission through discharge, completing assessments, developing individualized care plans, and collaborating closely with an interdisciplinary team to support functional recovery and independence. You will educate patients and families, connect them with resources, and ensure safe transitions of care. Manage a caseload of rehab patients from admission through discharge Coordinate individualized care plans with an interdisciplinary team Conduct assessments, monitor progress, and adjust care plans Educate patients and families on recovery goals and community resources Facilitate discharge planning and safe transitions of care Advocate for patients’ needs and remove barriers to care Maintain accurate, timely clinical documentation Collaborate with physicians, therapists, nurses, and social workers Ensure compliance with healthcare regulations and hospital policies Participate in quality improvement and team meetings Patient assessment Care coordination Rehabilitation care planning Electronic Medical Records (EMR) Discharge planning Motivational interviewing Utilization review and documentation Interdisciplinary team collaboration Knowledge of community resources Regulatory and compliance knowledge Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN, LSW, or related clinical license in applicable state Case Management certification (CCM, ACM, or equivalent) preferred BLS certification Experience in inpatient or outpatient rehabilitation setting preferred5c143e31-5e48-4549-b638-05792d185386
