OhioHealth is seeking a Clinical Rehab Liaison to drive growth of our rehabilitation services across our not-for-profit network. In this sales-focused clinical role, you will build and manage referral relationships with physicians, hospitals, and community partners while serving as a trusted resource for patients and families. Responsibilities include clinical screenings, coordinating admissions, educating stakeholders on rehab options, and tracking referral metrics. You’ll collaborate with interdisciplinary teams in a supportive, inclusive culture that values compassion, diversity, and innovation, with opportunities for ongoing professional development and community impact. Serve as primary liaison between referring providers, patients, and OhioHealth rehabilitation services Conduct clinical and functional screenings to determine appropriate rehab level of care Develop and manage referral relationships with physicians, hospitals, and community partners Educate providers, patients, and families on OhioHealth rehab programs and services Coordinate smooth patient admissions, authorizations, and transitions of care Track, analyze, and report referral and conversion metrics to meet growth targets Collaborate with clinical, case management, and finance teams to optimize patient access Represent OhioHealth at community events, outreach programs, and professional meetings Maintain accurate documentation within CRM and EMR systems Uphold OhioHealth’s values of compassion, diversity, equity, and patient-centered care Healthcare sales Clinical assessment Rehabilitation care coordination Referral development CRM software use Market analysis Physician relationship management Insurance/benefits knowledge Data reporting Presentation skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN, PT, OT, SLP, or LSW/LISW license in Ohio preferred or required per role specification Prior experience in rehabilitation, acute care, or post-acute care settings Valid driver’s license and reliable transportation for community outreach Familiarity with CMS and payer authorization requirements for rehab services Sales or account management experience in healthcare strongly preferred5c143e31-5e48-4549-b638-05792d185386
