Encompass Health seeks a compassionate Physical Therapist to provide evidence-based rehabilitation services in our inpatient rehab setting. You will evaluate patients, develop individualized treatment plans, and deliver therapeutic interventions to restore function, mobility, and independence. Collaborate closely with an interdisciplinary team to ensure patient-centered care, track progress, and adjust plans as needed. Educate patients and families on exercises, safety, and home programs. Evaluate patients’ functional status, mobility, and rehabilitation needs using standardized assessments Develop and implement individualized, evidence-based treatment plans and goals Provide therapeutic exercises, manual therapy, gait and balance training, and functional mobility interventions Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient-centered care Document evaluations, daily treatments, and progress notes accurately in the EMR system Educate patients and families on home exercise programs, safety, and strategies to maximize independence Participate in care conferences and discharge planning to support safe transitions of care Monitor patient progress, adjust treatment plans, and report changes in condition promptly Support quality, safety, and performance improvement initiatives within the rehab program Mentor and support PT students or new team members as needed Patient evaluation and assessment Development of individualized PT treatment plans Therapeutic exercise prescription Manual therapy techniques Gait and balance training Neuromuscular re-education Use of rehabilitation equipment and modalities Documentation and EMR proficiency Interdisciplinary team collaboration Patient and family education Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) or equivalent accredited PT degree Basic Life Support (BLS) certification ACLS preferred Experience with EMR systems Inpatient rehab or acute care rotation/experience5c143e31-5e48-4549-b638-05792d185386
