Encompass Health seeks a compassionate LPN to provide hands-on nursing care in our inpatient rehabilitation hospital. In this role, you’ll support patients recovering from major illnesses and injuries through medication administration, monitoring, and assistance with daily activities and mobility. Working within an interdisciplinary team, you’ll help implement care plans, educate patients and families, and ensure safety and infection control. Provide direct bedside nursing care to rehabilitation patients under RN and physician supervision Administer medications and treatments per provider orders and document accurately in the medical record Monitor, record, and report changes in patient condition, vital signs, and responses to therapy Assist with ADLs, mobility, and therapy activities to support functional recovery and safety Collaborate with RNs, therapists, and physicians in developing and following individualized care plans Educate patients and families on diagnosis, medications, rehabilitation goals, and discharge plans Maintain infection control, patient safety standards, and accurate documentation in compliance with policy Participate in interdisciplinary rounds, handoffs, and team meetings to coordinate care Support new staff by sharing unit protocols and contributing to a positive, team‑oriented culture Uphold ethical, patient‑centered care aligned with Encompass Health values of compassion and integrity Medication administration Vital signs monitoring Electronic health records (EHR) documentation Wound care and dressing changes Rehabilitation nursing principles Infection control procedures Patient and family education IV therapy and injections (per scope) Care planning support Basic life support (BLS) Active LPN/LVN license in state of practice Basic Life Support (BLS) certification IV certification where required by state or facility Experience in rehabilitation or acute care preferred5c143e31-5e48-4549-b638-05792d185386
