Holliswood Center, a skilled nursing and rehabilitation facility in Queens, NY, is seeking a compassionate Registered Dietitian. In this resident-centered role, you will complete comprehensive nutrition assessments, develop individualized care plans, and provide counseling to support residents’ recovery, health, and quality of life. You will collaborate closely with nursing, therapy, and medical teams, ensure regulatory compliance, and coordinate with food service to meet clinical and cultural needs. Perform comprehensive nutrition assessments for new and existing residents Develop, implement, and monitor individualized nutrition care plans Collaborate with nursing, therapy, and medical staff on interdisciplinary care Provide nutrition counseling and education to residents and families Monitor residents’ weight, intake, and clinical status; adjust care plans as needed Ensure compliance with regulatory, clinical, and documentation standards Participate in care conferences and quality improvement initiatives Coordinate with food service to align menus with clinical needs and preferences Document all assessments, interventions, and outcomes in medical records Educate staff on nutrition-related topics and special diets Clinical nutrition assessment Long-term care and skilled nursing nutrition Care plan development and management Diet modification and therapeutic diets Electronic health record (EHR) documentation Interdisciplinary team collaboration Regulatory and compliance knowledge (DOH, CMS) Nutrition counseling and education Menu planning and food service coordination Data tracking for weight and intake Geriatric and rehabilitation nutrition Time management and caseload prioritization Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Registered Dietitian (RD) credential Registered Dietitian Nutritionist (RDN) credential (preferred) State licensure as required by New York ServSafe or food safety certification (preferred) CPR certification (preferred)5c143e31-5e48-4549-b638-05792d185386
