U.S. Renal Care seeks a Kidney Care Options Educator / Care Coordinator to guide patients with chronic kidney disease through treatment choices and care planning. In this role, you will educate patients and families on dialysis, transplant, and home therapies; coordinate referrals and appointments; and collaborate with nurses, social workers, and physicians to support seamless care transitions. You will track patient progress, maintain accurate documentation, and provide compassionate support that reflects our patient-first culture. This position offers the opportunity to grow within a mission-driven, interdisciplinary kidney care team. Educate patients and families on chronic kidney disease and available treatment options, including dialysis, home therapies, and transplant. Coordinate referrals, appointments, and follow-up across clinics, hospitals, and community providers. Collaborate with nurses, physicians, social workers, and dietitians to develop and update patient-centered care plans. Maintain accurate, timely documentation in electronic health records and tracking systems. Monitor patient progress and barriers to care, escalating concerns to the clinical team as needed. Provide compassionate, culturally sensitive support and advocacy for patients and caregivers. Ensure compliance with HIPAA, organizational policies, and regulatory requirements. Participate in quality improvement and educational initiatives within the kidney care program.
