Senior Community Care of Northern Kentucky seeks a Registered Dietitian to support older adults through its Program of All-Inclusive Care for the Elderly (PACE). In this mission-driven role, you will assess participants’ nutritional needs, develop and monitor individualized nutrition care plans, provide counseling and education, and coordinate care with an interdisciplinary team. You will build trusting relationships with participants and families while helping frail seniors maintain health, dignity, and independence at home and in the community. Join a compassionate, person-centered team committed to safety, quality of life, and helping older adults age in place. Clinical or older-adult nutrition experience and an RD credential are expected. Assess participants’ nutritional status, dietary needs, and relevant health history. Develop individualized nutrition care plans in coordination with participants and the care team. Provide nutrition counseling and education to participants and families. Monitor nutrition outcomes and update care plans as needs change. Collaborate with interdisciplinary PACE team members to support participant health and independence. Document assessments, interventions, and progress in participant records.
