Senior Community Care of Northern Kentucky, a Program of All-Inclusive Care for the Elderly (PACE), is seeking a compassionate Registered Dietitian to support older adults in living safely and independently at home. You’ll assess participants’ nutritional needs, develop individualized care plans, provide counseling and education, and coordinate nutrition services with an interdisciplinary team. Build meaningful relationships with participants and caregivers while supporting dignity, safety, and quality of life. Join a mission-driven, collaborative team delivering comprehensive medical, social, and supportive care to frail seniors. Help participants manage health conditions, maintain appropriate nutrition, and age in place with confidence. Assess participants’ nutritional status, dietary needs, and health risks. Develop and update individualized nutrition care plans. Provide nutrition counseling and education to participants and caregivers. Coordinate nutrition-related care with the interdisciplinary PACE team. Support dietary needs associated with aging and chronic health conditions. Document assessments, care plans, interventions, and progress. Promote participant dignity, safety, independence, and quality of life.
