CommuniCare is seeking a Community Support Specialist to enhance resident well-being across our post-acute and long-term care settings. In this role, you will assess psychosocial needs, coordinate community and social services, and support residents and families through care transitions. You’ll collaborate closely with nursing, therapy, and leadership teams to create resident-centered plans, connect individuals with resources, and promote engagement in programs.
