Encompass Health seeks a compassionate Physical Therapist to deliver evidence-based rehabilitation services in our inpatient rehab hospital. You will assess, plan, and implement individualized treatment programs to restore mobility, function, and independence for diverse patient populations. Collaborating with an interdisciplinary team, you will provide therapeutic interventions, educate patients and families, document outcomes, and support safe discharges. Evaluate patients’ functional status, mobility, strength, balance, and pain levels. Develop and implement individualized treatment plans and therapy goals. Provide therapeutic exercises, gait training, neuromuscular re-education, and functional training. Educate patients and families on home exercise programs, safety, and adaptive strategies. Collaborate with physicians, nurses, and other therapists to coordinate care plans. Document evaluations, daily notes, progress reports, and discharge summaries accurately and timely. Participate in interdisciplinary team meetings and care conferences. Ensure compliance with regulatory, safety, and quality standards. Supervise and mentor PTAs and support staff as appropriate. Contribute to performance improvement and evidence-based practice initiatives.
