Encompass Health seeks a compassionate Physical Therapist to deliver evidence-based rehabilitation services in our inpatient rehab hospital. You will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to restore mobility, function, and independence. Collaborating closely with an interdisciplinary team, you’ll educate patients and families, document progress, and ensure safe discharges. Perform comprehensive physical therapy evaluations and functional assessments Develop and implement individualized, evidence-based treatment plans Provide therapeutic exercises, gait training, and balance and functional mobility training Educate patients and families on home exercise programs and safety Collaborate with physicians, nurses, and other rehab team members on care plans Document treatments, patient responses, and progress in the EMR accurately and timely Participate in discharge planning and recommend appropriate equipment and services Adhere to regulatory, ethical, and safety standards in all clinical care Contribute to quality improvement initiatives and clinical best practices Physical therapy evaluation and treatment planning Inpatient rehabilitation care Gait and balance training Neuromuscular re-education Orthopedic and post-surgical rehab techniques Functional mobility and transfer training Patient and family education Use of assistive devices and DME Electronic Medical Record (EMR) documentation Interdisciplinary team collaboration Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state licensure as a Physical Therapist (PT) Doctor of Physical Therapy (DPT) or equivalent accredited PT degree BLS (Basic Life Support) certification CPR certification Strong documentation and EMR proficiency Experience in inpatient rehab or acute care preferred5c143e31-5e48-4549-b638-05792d185386
