Encompass Health seeks a compassionate Physical Therapist to deliver high-quality, patient-centered rehabilitation services within our inpatient and home-based care network. In this role, you will evaluate functional limitations, develop individualized treatment plans, and provide evidence-based interventions to help patients regain mobility, strength, and independence. You will collaborate closely with an interdisciplinary team, educate patients and families, and document outcomes using EHR systems. Perform comprehensive PT evaluations and develop individualized treatment plans Deliver evidence-based therapeutic interventions to improve mobility, strength, and function Monitor, document, and report patient progress using EHR systems Educate patients and families on exercises, safety, and home programs Collaborate with physicians, nurses, and other rehab professionals on care plans Ensure compliance with regulatory, safety, and documentation standards Participate in interdisciplinary team meetings and discharge planning Contribute to quality improvement and best-practice initiatives Orthopedic and neurological physical therapy techniques Functional mobility and gait training Therapeutic exercise prescription and progression Manual therapy techniques Balance, vestibular, and fall-prevention interventions Patient and family education Use of rehabilitation equipment and assistive devices Electronic health record (EHR) documentation Interdisciplinary care coordination Outcome measurement and functional assessments Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Physical Therapist (PT) license in practicing state Doctor of Physical Therapy (DPT) or equivalent accredited PT degree Current BLS/CPR certification Experience with electronic health records (EHR) systems5c143e31-5e48-4549-b638-05792d185386
