Job Overview

This role is dedicated to equity in healthcare access and passionate about supporting community needs. The candidate must have strong organizational skills, the ability to manage a high volume of tasks, and adaptability in a fast-paced environment. Successful candidates will work collaboratively within a team, contributing to the creation of an inclusive and supportive environment that responds to the needs of the community. The Care Manager reports to the Chief Executive Officer and takes direction from the Behavioral Health Director and the Chief Operating Officer at HH & Co.


Key Responsibilities:

Enhanced Care Management Operations and Implementation - 30-40 hours/week


This role will start off with a smaller panel and will build to 20-25 clients within 2 months

  • Provide care management to patients of all ages who are connected through HH & Co. Behavioral Health or our community partners
  • Provide direct care to 25 patients - fewer based on acuity scaling
  • Proficient in Motivational Interviewing
  • Dedicated to Leading with Empathy
  • Maintain timely documentation and billing practices
  • Support the development and implementation of ECM line of service at HH & Co.
  • Collaborate with Leadership Team in the development of program protocols, referral processes, documentation, claims submission
  • Support the development of the field guide

Qualifications & Skills:

  • A background or current pursuit of a career in therapy or counseling is preferred, though not required
  • 2 years of direct client/patient service experience
  • Strong emotional intelligence (EQ) with the ability to understand and respond to the needs of both patients and team members with empathy, confidentiality and professionalism
  • Strong communication skills, both written and verbal, with a focus on clarity, professionalism, and responsiveness


Pay Range:


This is a full-time position (32-40 hours) in-person at our office in downtown Santa Cruz and in the field. The compensation package will be dependent on experience, qualifications, and the interview with a potential range of up to $26-33 per hour.
