Job Overview

The Care Manager reports to the Chief Executive Officer and takes direction from the Behavioral Health Director and the Chief Operating Officer at HH & Co.
Key Responsibilities:
Enhanced Care Management Operations and Implementation - 30-40 hours/week
This role will start off with a smaller panel and will build to 20-25 clients within 2 months

Pay Range:
This is a full-time position (32-40 hours) in-person at our office in downtown Santa Cruz and in the field. The compensation package will be dependent on experience, qualifications, and the interview with a potential range of up to $26-33 per hour.
