Providers with settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges
The RFA, CV, and 24 months of relevant activity is needed
A completed application separate from / following the above step
References on standby to respond immediately

Rate is higher for this need as well.

• Schedule Type: Hospital and Call
• Call Type: 24hr weekend call (beeper)
• Schedule (per week): please see specific dates need below

10/26 - 10/29: 7a-3p
10/30 - 11/1: 24- hr call shifts

• Dates of Need: see specific dates above & shift times
• Shift Times: above
• Lunch N/A unpaid
• Provider Min # Shifts Required: shifts outlined
• Guaranteed Hours per Day: 8
• Hours Worked for OT to apply: 8
• Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.

• Boards: BC or truly BE

ADDITIONAL DETAILS

Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):

NOT reimbursed:
o Mileage from provider's home to departing airport, as well as from airport to home upon return. o Ride share (Lyft/Uber/etc.) and/or taxi from provider's home to departing airport, as well as from airport to home upon return.

WILL be reimbursed:

o Airport parking at provider's departing airport if provider drives themselves to the airport. o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed. o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trip
