San Gabriel/Pomona Regional Center (SGPRC) is seeking a dedicated Service Coordinator to join our team, dedicated to facilitating and managing services for individuals with developmental disabilities. In this role, you will act as a liaison between our clients, their families, and service providers, ensuring seamless coordination and optimal support. This position requires a compassionate professional capable of assessing client needs, developing individualized service plans, and monitoring outcomes to enhance the quality of life for those we serve. Conduct initial needs assessments for new clients and develop personalized service plans. Regularly review and modify service plans in response to changing needs of clients. Coordinate with service providers to ensure that clients receive necessary services. Assist families and clients in understanding available resources and support services. Monitor and evaluate the effectiveness of services provided to clients and make adjustments as necessary. Maintain accurate and detailed client records in compliance with regulatory requirements. Provide crisis intervention as needed and coordinate with other professionals to provide comprehensive support.
