Requirements

Requirements:

• Minimum 2 years of acute care Social Work experience
• Active California LCSW license
• BLS certification required
• California 5150 Certification required
• Previous California Dignity Health experience is required
• Experience with Cerner electronic medical record preferred
• Strong knowledge of discharge planning, crisis intervention, care coordination, and psychosocial assessments
• Excellent communication and interdisciplinary collaboration skills
• Ability to work independently in a fast-paced acute care environment
