The Home Infusion Pharmacist is responsible for the safe and accurate preparation, dispensing, and monitoring of intravenous (IV) medications administered in a patient's home or alternate care setting. This role ensures compliance with all regulatory standards while collaborating with healthcare providers to optimize patient outcomes in a home infusion environment.


Key Responsibilities:

  • Review and verify physician orders for home infusion therapies, including antibiotics, total parenteral nutrition (TPN), chemotherapy, and specialty biologics
  • Compound and supervise the preparation of sterile IV medications in accordance with USP and standards
  • Ensure accurate dispensing and labeling of medications and supplies for home delivery
  • Provide clinical oversight, including dosing adjustments, therapeutic monitoring, and patient-specific care planning
  • Collaborate with physicians, nurses, and case managers to coordinate patient care
  • Counsel patients and caregivers on medication administration, storage, side effects, and safety
  • Monitor patient outcomes and laboratory results; intervene as necessary
  • Maintain compliance with state and federal regulations, accreditation standards, and company policies
  • Oversee pharmacy technicians and support staff in compounding and workflow processes
  • Participate in on-call rotation for clinical support and urgent patient needs

Qualifications:

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
  • Active pharmacist license in the applicable state
  • Experience in sterile compounding and home infusion or hospital pharmacy preferred
  • Knowledge of USP , USP , and aseptic technique standards
  • Strong clinical knowledge of IV therapies and disease states
  • Excellent communication and organizational skills

Preferred Qualifications:

  • Board certification (e.g., BCPS)
  • Experience with home infusion pharmacy services
  • Familiarity with electronic medical records and pharmacy management systems

Work Environment:

  • Pharmacy cleanroom and office setting
  • May include occasional patient interaction (in-person or virtual)
  • On-call or weekend rotations may be required

Physical Requirements:

  • Ability to stand for extended periods
  • Manual dexterity for sterile compounding
  • Ability to lift and transport medication supplies (up to 25 lbs)
