We are currently seeking a compassionate and dedicated full-time Clinical Supervisor to join our in-office team! As a Clinical Supervisor, you will play a crucial role in maintaining high-quality patient care, through planning, coordination, and implantation and evaluation of the Home Health services provided. The Clinical Supervisor is available, at all times, during operating hours.
Responsibilities:
- Coordinate and schedule the daily workload of each clinician, utilizing Agency personnel to their fullest capacity.
- Provide daily supervision of district nurses, and home health aides and therapists, to ensure the quality of services and maintenance standards.
- Review and investigate the clinical content, diagnoses, medical history, medications, fee status, and other procedures, from the patient records.
- Direct staff towards the appropriate use of community resources
- Appraise staff performance, during their probationary period, and annually, with the DON and/or Executive Director; provide the teaching and counseling necessary to enhance the delivery of services, and the professional development of nursing and Health Aide staff.
- Identify, and evaluate, patient needs and appropriateness of home care, by reviewing the initial referral and intake forms, before assignment of direct care staff.
- Participate in community related functions, as necessary
- Assist with the orientation of new clinical staff, to familiarize them with agency policies and procedures
- See patients in the home setting as needed.
- Participate in audits, utilization review interdisciplinary team conferences and represent Agency, in the community, as requested
- Review each clinician scheduled for recertifications, Discharges and Health Aide supervisory visits.
- Review weekly schedules of clinicians for changes/productivity.
- Generate reports regarding patients' re-hospitalization, referrals, admissions, and recertifications daily and confer with intake and scheduling regarding recertifications as well as other patient care and scheduling needs.
- Perform other duties as directed.by Administrative Director or Director of Nursing.
- May be requested by Executive Director or Director of Nursing, to fill in for other clinical staff when necessary.
Qualifications:
- Must be a Registered Nurse licensed, by the state of Florida.
- Must be a graduate of an accredited School of Nursing.
- Must possess a polite telephone manner.
- Must be able to interact with health professionals at all levels
- Must have a minimum of 2 years Home Health Nursing, is preferred.
- Must have a driver's license and a reliable means of transportation to ensure timely arrival at patient's homes.
- Must be organized and detail oriented.
- Prior utilization of PC, excel, word documents, calculator, multi-line telephone, copy machine, and basic nursing equipment is strongly preferred.
- Must have a criminal background check.
- Must have a current CPR certification.
Job Conditions:
- Position may be stressful in terms of meeting deadlines.
- It requires minimal lifting of office records and printouts.
- It is primarily a desk job, which essentially involves sitting, standing stooping, and walking, as well as an inordinate amount of telephone communication.
- Travel is required, by car or airplane to local, out-of-town, or state seminars, conferences, or meetings.
- The ability to communicate well, both verbally and in writing, in English is required.
- Must be able to read type that is 12 point or larger.
- The ability to access patients' homes, which may not be routinely wheelchair accessible, is required.
- Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
- On occasion, one may be required to bend, stoop, reach, and move patients up to 250 lbs and lift/carry up to 30 lbs
- The ability to access patients' homes, which may not be routinely wheelchair accessible, hearing and eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care is required.
