Encompass Health seeks a Rehabilitation Physical Therapist to deliver evidence-based, patient-centered care in our inpatient rehabilitation hospital. You will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to restore mobility, strength, and function. Collaborate closely with nurses, physicians, and the interdisciplinary rehab team to support safe discharges and optimal outcomes. Evaluate patients’ functional status and develop individualized rehab plans of care Deliver evidence-based physical therapy to restore mobility, strength, and balance Provide gait, transfer, and functional mobility training in an inpatient rehab setting Educate patients and families on exercises, safety, and home programs Document assessments, goals, and progress in the EMR accurately and timely Collaborate closely with RNs, physicians, and the interdisciplinary rehab team Participate in discharge planning to support safe transition of care Contribute to a nurturing, patient-centered culture and quality initiatives Inpatient rehabilitation therapy Therapeutic exercise prescription Gait and balance training Neuromuscular re-education Orthopedic and neuro rehab treatment planning Functional mobility and transfer training Patient assessment and care planning Electronic medical record (EMR) documentation Patient and family education Interdisciplinary team collaboration Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) or accredited PT degree BLS/CPR certification Experience in inpatient rehabilitation preferred5c143e31-5e48-4549-b638-05792d185386
