Aspire Health Partners, a leading nonprofit behavioral healthcare provider in Central Florida, is seeking a Community Based Counselor to deliver client-centered counseling and recovery-focused support in home and community settings. In this role, you will provide mental health and substance use assessments, facilitate individualized treatment and safety plans, and deliver evidence-based interventions for children, adults, and families. You will coordinate care with multidisciplinary teams, document in EHR systems, and link clients to community resources. Provide community-based counseling and support to clients with mental health and substance use needs Conduct biopsychosocial assessments, risk evaluations, and safety planning Develop and implement individualized, recovery-focused treatment plans Deliver evidence-based interventions for individuals and families in home and community settings Coordinate care with psychiatrists, nurses, case managers, and community partners Maintain accurate, timely documentation in electronic health records (EHR) Educate clients and families on coping strategies, recovery, and available resources Facilitate referrals and linkages to community services and supports Participate in team meetings, supervision, and ongoing training Uphold Aspire’s mission, cultural values, and regulatory and ethical standards Community mental health counseling Substance use counseling Case management Risk assessment and safety planning Crisis intervention Motivational interviewing Electronic health records (EHR) documentation Treatment and recovery planning Care coordination and referrals Client and family psychoeducation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed Mental Health Counselor (LMHC) or eligibility preferred Licensed Clinical Social Worker (LCSW) or eligibility preferred Licensed Marriage and Family Therapist (LMFT) or eligibility preferred Valid Florida driver’s license and reliable transportation CPR/First Aid certification preferred Background clearance per Florida health regulations5c143e31-5e48-4549-b638-05792d185386
