Licensed Clinical Social Worker

Salary: Up to $104,000 annually (depending on experience)

About Us:

We are seeking a dedicated and empathetic Medical LCSW to join our interdisciplinary team and support patients and families through complex medical journeys.

Medical LCSW Key Responsibilities:

• Provide psychosocial assessments and counseling to patients and families in medical settings (inpatient, outpatient, or home health).
• Collaborate with physicians, nurses, and case managers to develop and implement care plans.
• Assist with discharge planning, resource coordination, and crisis intervention.
• Advocate for patients' mental health and social needs within the healthcare system.
• Maintain accurate and timely documentation in the EHR.

Medical LCSW Qualifications:

• Master's degree in Social Work (MSW) from an accredited program.
• Current Hawaii LCSW license in good standing.
• Minimum 2 years of experience in a medical or hospital setting preferred.
• Strong knowledge of community resources, insurance, and care coordination.
• Knowledge of Medicare guidelines.
• Excellent communication, empathy, and problem-solving skills.
