Kaiser Permanente seeks a Hospital Case Manager to coordinate patient care and discharge planning in a tech-forward environment. You’ll leverage modern healthcare IT tools to assess needs, build care plans, and connect patients with appropriate services. Working closely with clinical teams, you’ll manage authorizations, ensure continuity of care, and support patients and families with education and resources.
