Carle Health is seeking a dedicated Dietitian to provide evidence-based nutritional care to patients in a community-focused healthcare setting. In this role, you will assess nutritional needs, develop individualized care plans, and collaborate closely with physicians, nurses, and other care team members to improve health outcomes. You will educate patients and families on diet, chronic disease management, and lifestyle changes, while maintaining accurate documentation in the electronic health record. Assess patients’ nutritional status using medical history, labs, and dietary intake Develop and implement individualized nutrition care plans Provide medical nutrition therapy for chronic and acute conditions Educate patients, families, and caregivers on diet and lifestyle changes Collaborate with physicians, nurses, and allied health professionals Document assessments, plans, and outcomes in the EHR accurately and timely Participate in interdisciplinary rounds and care conferences Contribute to quality improvement and clinical best-practice initiatives Ensure compliance with regulatory and accreditation standards Stay current with evidence-based nutrition research and guidelines Nutritional assessment Medical nutrition therapy (MNT) Clinical documentation/EHR Patient and family counseling Menu and diet planning Interdisciplinary collaboration Knowledge of chronic disease management Anthropometric and lab data interpretation Regulatory and compliance knowledge (JCAHO, CMS) Time management and caseload prioritization Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Registered Dietitian Nutritionist (RDN) credential preferred or required State licensure as a Dietitian/Nutritionist where applicable Certification in specialized areas (e.g., CDE/CDCES, CNSC) a plus Basic Life Support (BLS) certification preferred Experience with electronic health records (EHR)5c143e31-5e48-4549-b638-05792d185386
