This is a full-time on-site role for a Physical Therapist located in Greater Indianapolis. The Physical Therapist will be responsible for assessing patients' physical conditions, developing personalized treatment plans, and providing effective rehabilitation services. Core tasks include assisting patients in restoring movement and functionality, reducing pain, and preventing disability. The role also involves monitoring patient progress, educating patients and families about care plans, and collaborating with other health care professionals to ensure optimal care.

• Strong expertise in Physical Therapy and Rehabilitation techniques
• Experience in Patient Care and delivering quality Home Care services
• Knowledge of Medical principles as they relate to physical therapy
• Excellent interpersonal, communication, and problem-solving skills
• Ability to work collaboratively with a multidisciplinary team
• Valid state licensure or eligibility for licensing as a Physical Therapist
• Bachelor's or Doctor of Physical Therapy (DPT) degree from an accredited program
• Previous experience in home health care is a strong advantage
