Senior Community Care of Northern Kentucky, a Program of All-Inclusive Care for the Elderly (PACE), is seeking a Registered Dietitian to support older adults in remaining safely and independently at home. Provide nutrition assessments, individualized care plans, medical nutrition therapy, and practical education tailored to each participant’s health needs, preferences, and goals. Monitor nutritional status and coordinate interventions with participants, caregivers, and the interdisciplinary care team. Build meaningful relationships while helping frail seniors maintain quality of life and age in place. Registered Dietitian Nutritionist credentials and Kentucky licensure or eligibility are expected. Assess participants’ nutritional status, dietary intake, and nutrition-related risks. Develop and update individualized nutrition care plans in coordination with the interdisciplinary team. Provide medical nutrition therapy, counseling, and education to participants and caregivers. Monitor nutrition outcomes and adjust interventions as health needs change. Coordinate nutrition services and communicate recommendations to care team members. Document assessments, care plans, interventions, and progress in participant records.
