Senior Community Care of Northern Kentucky is seeking a Registered Dietitian to support older adults enrolled in its Program of All-Inclusive Care for the Elderly (PACE). You will assess participants’ nutritional needs, develop and monitor individualized nutrition care plans, provide counseling and education, and coordinate care with an interdisciplinary team. Partner with participants, families, and colleagues to address health needs while respecting each person’s preferences, dignity, and independence. This role offers meaningful relationships and the opportunity to improve participants’ quality of life. Assess participants’ nutritional status, dietary needs, and health-related nutrition concerns. Develop, implement, and monitor individualized nutrition care plans. Provide nutrition counseling and education to participants and caregivers. Coordinate nutrition goals and services with the interdisciplinary PACE care team. Document assessments, recommendations, and progress in participant records. Support older adults in maintaining health, dignity, and independence in their homes and communities.
