Aveanna Healthcare seeks an In Home Healthcare LPN to provide high-acuity skilled nursing care to pediatric and adult patients in their homes. Responsibilities include comprehensive assessments, medication administration, trach/vent and wound care, and close monitoring of complex conditions. You’ll collaborate with an interdisciplinary team, follow individualized care plans, document in EMR, and educate families on ongoing care. Provide in-home skilled nursing care for high-acuity pediatric and adult patients Perform comprehensive assessments and monitor vital signs and patient status Administer medications and treatments per provider orders Provide tracheostomy, ventilator, and wound care as needed Follow individualized care plans and update the clinical team on changes Document care accurately in EMR and maintain clinical records Educate patients and families on disease management and home care procedures Collaborate with interdisciplinary team to optimize patient outcomes Adhere to infection control, safety, and regulatory standards Participate in ongoing clinical training and competency development LPN clinical assessment Medication administration Wound care Tracheostomy care Ventilator management Documentation in EMR Care planning Infection control Patient and family education Vital signs monitoring Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN license in state of practice BLS/CPR certification Experience with tracheostomy and ventilator care preferred Pediatric or high-acuity home health experience preferred5c143e31-5e48-4549-b638-05792d185386 read more
