Encompass Health is seeking a dedicated Rehab Liaison to join our dynamic team. In this role, you will act as the vital link between our healthcare services and the patients in need of rehabilitation. By effectively collaborating with hospital staff, physicians, and patients, you will ensure smooth transitions and clear communication throughout the rehabilitation process. Our Rehab Liaisons help shape the patient’s first impressions of our integrated healthcare services, making this role crucial for maintaining the high standard of care Encompass Health is known for across 42 states. Coordinate and streamline the transfer of patients from healthcare facilities to our rehabilitation services. Educate patients and their families about the services at Encompass Health. Collaboratively work with healthcare professionals to determine the most appropriate rehabilitation services for patients. Maintain accurate and timely records of activities and patient interactions. Contribute to the development of outreach and education programs.
