Element Care, a nonprofit PACE organization, is seeking a Registered Dietitian to provide comprehensive nutrition care to frail seniors in community-based and center settings. The RD completes nutrition assessments, develops individualized care plans, and collaborates with an interdisciplinary team to support safe, independent living. Responsibilities include counseling participants and families, monitoring special diets, documenting in the EHR, and contributing to case conferences. Complete comprehensive nutrition assessments for PACE participants Develop and update individualized nutrition care plans Provide one-on-one and group nutrition counseling to seniors and families Monitor therapeutic and modified diets, supplements, and weight trends Document assessments, plans, and outcomes in the electronic health record Participate in interdisciplinary team meetings and case conferences Collaborate with physicians, nurses, social workers, and therapists on care goals Educate participants and caregivers on chronic disease and nutrition management Support quality improvement initiatives related to nutrition and senior health Ensure compliance with regulatory and organizational nutrition standards Medical nutrition therapy Geriatric nutrition Nutritional assessment Care planning Interdisciplinary collaboration Electronic health records (EHR) Patient and family education Motivational interviewing Regulatory documentation Time management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Registered Dietitian (RD) credential Active Massachusetts RD license CPR/BLS certification Certified Diabetes Care and Education Specialist (CDCES) preferred Experience with electronic health records (EHR) preferred5c143e31-5e48-4549-b638-05792d185386
