Encompass Health seeks a Certified Case Manager to coordinate inpatient rehabilitation cases from admission through discharge. In this role, you’ll complete assessments, develop care plans, and collaborate with an interdisciplinary team to support recovery after major illness or injury. You’ll arrange post-discharge services, ensure safe transitions of care, and advocate for patients and families. Working in a patient-centered, team-oriented environment, you’ll contribute to quality improvement while accessing robust training, professional development, and career advancement opportunities.
