Christian Park Healthcare Center seeks a compassionate Registered Dietitian to support residents in our skilled nursing and rehab setting. The Dietitian will assess nutritional needs, develop and monitor individualized care plans, collaborate with nursing, therapy, and medical staff, and provide education to residents and families. This role supports quality outcomes, clinical excellence, and resident dignity by promoting optimal nutrition, specialized diets, and safe swallowing. Join our interdisciplinary team and help create a homelike, supportive environment that enhances residents’ health, independence, and quality of life. Assess residents’ nutritional status through clinical evaluations and chart reviews Develop, implement, and update individualized nutrition care plans Recommend and monitor therapeutic and texture-modified diets and supplements Collaborate with nursing, therapy, and medical staff on care planning and quality outcomes Document assessments, interventions, and progress per regulatory and facility standards Provide nutrition education to residents, families, and staff Participate in care conferences, QAPI, and regulatory surveys as needed Monitor weight trends, intake, and nutritional risk indicators Support safe swallowing plans in collaboration with Speech Therapy Ensure compliance with state, federal, and facility nutrition standards Medical nutrition therapy Long-term care nutrition Nutritional assessment Care planning and documentation Therapeutic diets Texture-modified diets and dysphagia management Malnutrition identification and intervention Interdisciplinary team collaboration Regulatory compliance (CMS, state) Electronic medical record (EMR) documentation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Registered Dietitian (RD/RDN) credential State of Michigan dietitian/nutritionist license (or eligibility) ServSafe or food safety certification preferred Experience in skilled nursing or long-term care preferred5c143e31-5e48-4549-b638-05792d185386
