JOB SUMMARY Staff Family Nurse Practitioner provides services to the clients at our facility and in the community through outreach and community programs. Participates as a member of the Patient Centered Health Home (PCHH) Care Team. Populations Served: Pediatric (2 - 12 yrs.), Adolescent (13 - 19 yrs.), Adult (20 - 64 yrs.), Geriatric (65 yrs. & older) GENERAL REQUIREMENTSProvides direct outpatient primary care to patients by performing diagnostic and therapeutic services to assist physician in diagnosing medical and health and prescribing necessary treatment for primary patient care; records pertinent data for review by physicians.Takes patient history; conducts physical exams, records data; writes progress notes and narrative summaries.Prescribes medication utilizing clinic formulary for outpatients as feasible and per physician assistant protocols.Acts as the liaison with medical care resources when referral is needed per protocolsRequisitions on and off-site lab and x-ray exams as needed per protocol.Manages chronic disease utilizing established protocols and procedures.Participates in continuing education activities on- and off-site when appropriate and feasible.Participates in staff conferences to devise individual patient care plans, to revise and update clinic services, continuing quality improvement program and continuing education as appropriate.When there are no patients to be attended to, participates in patient related activities.Assist the patient in the development of patients self-management goals and care plans. Documents patients progress with meeting goals.Focus on assessment, monitoring and proactive management of the healthcare needs of the patient populationParticipate on the Patient Centered Health Home (PCHH) Care Team.Participate in PCHH education on the function of self-management tools and techniques.Participates in other related activities affecting the clinics programs. CERTIFICATIONS AND LICENSURES Cardiopulmonary Resuscitation (CPR) Full licensure to practice medicine by the NC Board of Nursing, with valid license.
