As a Case Manager at Altru Rehabilitation Hospital, you will coordinate individualized rehabilitation care plans to support patients in regaining function, independence, and quality of life. You’ll complete comprehensive assessments, monitor progress, and collaborate closely with physicians, therapists, nurses, and social workers to ensure seamless, patient-centered care. In this on-site role, you will educate and advocate for patients and families, facilitate safe discharge planning, connect them with community resources, and maintain accurate documentation in our electronic health record. Advocate for patient needs, ensuring access to services and support systems. Document assessments, care plans, and progress notes in the electronic health record.
